Nottinghamshire firm Van Elle has been fined by the Health and Safety Executive (HSE) after an employee suffered an accident at work.
The unnamed worker was operating a wheeled excavator during widening work on the M1 motorway when the machine struck a bridge.
An investigation found the employee had not received adequate training in how to use the excavator and was standing in for a regular driver.
He was not wearing his seatbelt and, as a result, was thrown over the steering column through the front screen, hitting his head on the excavator blade.
The incident resulted in the staff member falling into a coma for two weeks with severe head injuries, followed by a five-month period of rehabilitation.
HSE inspector Kevin Wilson said: "These injuries were wholly preventable had the company ensured the driver had adequate training in safe travel positions for manoeuvring the excavator on the construction project."
The news comes after Spectra Scaffolding was prosecuted by the body following an incident which led to HGV driver David Collins being left without the use of his legs after metal tubes fell from a crane and crushed him.
