A father-of-two tragically lost his life in an accident at work at a Somerset Quarry.
Neal Webber was killed while carrying out maintenance in a large limestone fines hopper at Whatley Quarry in Frome in 2008.
The 28-year-old and colleague Neal Myers were removing compacted material from the inside of the hopper when the material they were standing on collapsed.
While Mr Myers was buried up to his chest, he managed to pull himself free. Mr Webber, however, was buried under approximately 12 tons of limestone dust and was suffocated.
Both men were wearing harnesses, but the equipment used by Mr Webber did not prevent him from being buried under the limestone dust, while Mr Myers' shorter line meant he was not fully buried.
Health and Safety inspector Ann Linden commented: "Because of this tragic incident two young children have been left without a father and Mr Myers was left traumatised by being buried up to his chest in limestone dust and being unable to rescue his workmate."
