Two construction firms have been fined after a man died in a tragic accident at work.
Peter Cole was replacing the lights on a central reservation on a Liverpool duel carriageway when the arm of the cherry picker he was standing on collapsed.
The 61-year-old fell almost eight metres and landed on the back of the vehicle. He died in hospital later that day.
Amey Infrastructure Services Ltd and Mouchel Parkman Services Ltd were both fined £30,000 and ordered to pay £32,000 in prosecution costs.
Health and Safety Executive inspector Dave Guyers commented: "All companies must have effective checking and maintenance systems in place and act upon the findings from them. Ageing machinery should be maintained properly and responsibly - not doing so increases the likelihood of this sort of incident."
In other news, Neales Waste Management Ltd was fined £15,000 and ordered to pay £11,661 in costs after an employee fell from the roof of a truck in an accident at work, sustaining a broken right elbow and damage to his left foot.
