Two firms have been a total of £300,000 after an employee was killed in an accident at work on the M5 motorway.
Cecil Grant, 42, from Southmead, Bristol, was repairing CCTV cameras when he fell 12 metres off a wall. He died ten days later in hospital.
Following the incident in 2006, the Health and Safety Executive (HSE) prosecuted Mr Grant's employers Serco Ltd, of Bartley Wood Business Park, Hook, Hampshire, and principal contractor, Birse Civils Ltd of Grimstone Grane, Tadcaster, North Yorkshire for failing to protect Mr Grant when he was working at the site.
HSE inspector Steve Frain said "There were major failings in the way this work was planned which sadly resulted in Mr Grant's tragic death.
"When employees are working at height, proper plans must be put in place and workers made aware of possible risks."
In 2008/2009, there were over 4,000 major injuries caused by falls from height at work.
