Two companies have been fined a total of £640,000 after two people were killed in an accident at work.
In May 2009, Maarten Den Heijer and Robert MacDonald died after going to the rescue of a colleague at a salmon farm on Loch Creran, Arygll and Bute.
The two men, who worked for Scottish Sea Farms, were attempting to help co-worker Campbell Files, who had passed out on a barge moored on the loch when he went below deck to find equipment. Oxygen levels below deck were dangerously low.
They had been summoned by Arthur Raikes of Logan Inglis Limited, who had gone below deck with Mr Files but managed to struggle out.
Inspectors from the Health and Safety Executive (HSE) discovered that neither firm had provided suitable information, instruction and training for employees working in the small sealed chambers on the Loch Creran barge.
HSE principal inspector Barry Baker said: "The deaths in this case should have been avoided - the risks should have been identified and a clear and safe system of work prepared."
Scottish Sea Farms was fined £600,000, while Logan Inglis Limited was ordered to pay £40,000.
It comes after HSE figures show a rise in the number of people killed at work in the last year.
