A Buckinghamshire plant hire company has been prosecuted after a fatal accident at work.
Morgans Plant Hire Ltd was fined £50,000 after a worker was killed when a forklift truck he was driving overturned.
Keith Napping, 42, died at the site of the incident, which occurred in 2007.
A Health and Safety Executive (HSE) investigation revealed the company failed to ensure the forklift truck was in good working order.
At Aylesbury Magistrates' Court, a mechanic working for the firm was also fined after he incorrectly modified the brakes of the vehicle.
HSE inspector Peter Burns said: "This prosecution serves to show just how important it is for companies to take their health and safety obligations seriously. HSE will not hesitate to prosecute any firm that fails to do so."
Meanwhile, the owner of Flooks Scaffolding in Bristol has been fined after a worker died when he fell from scaffolding.
Russell Lee Flook was ordered to pay £3,000 after he pleaded guilty to breaching health and safety regulations.
