Two aerospace firms have been fined a total of £75,000 after an accident at work that saw an employee crushed to death at a factory in Darwen.
Allan Sanderson was pushing a trolley carrying more than two tonnes of steel when it collapsed onto him. He later died in hospital.
Brookhouse Composites and Brookhouse Tooling were prosecuted by the Health and Safety Executive (HSE) following the incident in 2008.
Another worker, Gerald Powderley, was also hurt in the accident at work, breaking both his legs, his right foot and ankle.
The HSE investigation revealed that the trolley containing the steel was too narrow to fit on load-bearing rails, which left one set of wheels being pushed along a pedestrian walkway that was not strong enough to support the weight.
Speaking after the hearing, Mr Sanderson's widow Lorraine said: "I find it difficult coming to terms with the fact that Allan went to work one day but never came home. I just wouldn't like to think of the same thing happening to someone else."
It comes as the government proposes to reduce the number of workplace safety visits by a third, which unions say will mean more accident claims.
