A Hammersmith firm has been fined after an accident at work that later led to an employee having his leg amputated.
Twenty-three-year-old Lukasz Taborek from Acton in Ealing fell more than three metres when the floor he was working on collapsed.
Falling materials landed on him, completely crushing his right foot and later leading to the amputation of his lower right leg.
A Health and Safety Executive (HSE) investigation found that the contractor Urang Ltd, now known as Howper 291 Ltd, had assigned a project manager with no health and safety training.
It was also revealed in court that the first floor, on which Mr Taborek was working, had been incorrectly built by sub-contractors.
HSE inspector Peter Collingwood said:"This incident was entirely preventable. Had there been adequate management and supervision of the site, this terrible incident could have been avoided."
The news comes as John Handley, a partner in Nottingham construction firm J&C Handley, is fined after a sub-contractor suffered permanent leg injuries in a fall.
